Peterborough City Council’s Transfer of Care Team are developing an exciting way of working and have a fantastic opportunity for Adult Social Worker, to join our supportive and diverse team. You will preferably have some social work experience and can work as part of a team.

We are looking for a dynamic, committed, and passionate individual who is dedicated to improving people’s lives, working alongside other professional colleagues to promote the principles of personalisation, encouraging independence, choice and opportunities.

To be successful in this role, you will have knowledge, experience and understanding of the needs of vulnerable adults. Working knowledge and experience of the Care Act 2014, Mental Capacity Act 2005, and Human Rights Act 1998. You will also have an understanding of National and local polices and guidance relating to Hospital discharges.
