To work in the new Short Term Adult Reablement Service (STARS*)

The Short Term Adult Reablement Service (STARS*) consists of mental wellbeing STAR* workers and a Manager and is a new community mental wellbeing service. It is designed to support motivated people of 18 years or over, living within PCC boundaries, wanting to maximise their resilience and increase their independence at home/in the community.

The service aims to enable people to choose and attain meaningful connections such as friendships, activities, hobbies and interests, in their community. The service will focus exclusively on Care Act (2014) eligible social care issues. We support people to regain confidence and skills they may have lost as a consequence of poor health, being in hospital or being isolated, which may have affected their mental wellbeing and ability to perform their usual activities of daily living. The STARS* team offers a stepping stone of intensive, personalised reablement for motivated people choosing to work with us to rebuild their resilience and regain their skills, enabling them to live as independently as possible in their own homes, reconnecting to a life which is meaningful to them. The STARS* team will work together with the person in a collaborative partnership towards achieving the person’s unique goals, promoting independence and recovery. Working with adults from 18 years upwards.

We are looking to recruit an enthusiastic person to be part of a team, willing to work in a flexible way, who is motivated to support people with mental health problems to become more independent. This is an exciting opportunity for the right candidate to be part of the new STARS* and be part of the developing service for the future of mental health services locally.

Full time post - Monday to Friday, 37 1/2 hours, 9am to 5pm
Part time post - 30 hours per week (salary will be pro rata equivalent of full time post)
