We have a highly rewarding opportunity within the Transfer of Care Team in Peterborough City Hospital, where you will promote the independence of adults and older people in need of care services and their carers by assessing their needs, promoting reablement, providing equipment and support and arranging personal budgets or person-centred care services for those with long term needs.


You will promote personalised care pathways ensuring choice and control for all eligible individuals; and work with others to influence and shape the local environment, to promote social inclusion and accessibility and ensure that identified needs are understood individually, by locality and fed into strategic commissioning.

About you

Our ideal candidate should be self-motivating, high achiever, dynamic and innovative; and able to work as part of a team, work under pressure and meet strict deadlines and targets.


It is crucial that you are able to work with complex or challenging cases with appropriate support and supervision - where there may be issues of vulnerability and risk and where crisis intervention may be required in compliance with health and safety policies.


We do ask that you have in-depth knowledge and understanding of social care legislation and regulation and how to interpret this in a modern context, including:

•Multi-disciplinary complex casework

•CQC Care Standards

You should be educated to Dip SW or CQSW, must be registered with relevant body (e.g. SWE) or Foreign qualification where certified by General Social Care Council) and be willing to undertake further training.
