The 0 to 25 Disability Team work with disabled children, young people transitioning into adult services and adulthood and their families. The service operates as one service across 2 Teams; 0 to 18 and 14 to 25. We have an exciting opportunity to offer for an experienced social worker with an interest in and /or experience of working with Children and Young People with Disabilities.

The Advanced Practitioner post offers an opportunity for an experienced social worker to come and work in a very supportive team. The team are committed to supporting children and young people to ensure they have opportunities to achieve their aspirations.

The role will support members of the team who are less experienced with their continued development and the successful candidate will have opportunities to develop their own practice, knowledge and skills through training and mentoring.

The hours are either 30 (pro rata) or 37 hours per week.
