We are looking for an Occupational Therapist to join our Reablement service within the Home Services Delivery Model.
We are looking for an experienced Occupational Therapist to join our busy Reablement service.
Our service is Therapy led with clients receiving Full holistic assessments within the clients’ home and completing Reablement Goal setting in collaboration with the client.
You will be required to:
- Manage your own caseload effectively and efficiently
- work at a fast pace ensuring we meet time-frames of assessment within 48 hours of service commencement with clients
- provide appropriate equipment and minor adaptations
- complete timely records and reports
- working within a multidisciplinary team to support the running of the reablement service
