The Reablement Department is seeking a dedicated Reablement Hospital Coordinator to support the successful discharge of clients from acute hospital settings and ensure they receive the best possible support following their hospital stay

Key Responsibilities:

· Support clients and their families with information and guidance on the reablement process.

· Work closely with the Hospital Discharge team to ensure seamless communication and strong relationships between health colleagues and adult social care.

· Be highly organized to complete all tasks accurately and on time.

· Maintain a confident and pleasant telephone manner, even in a busy and stressful environment.

· Work under pressure against tight deadlines to ensure service delivery.

Requirements:

· Strong problem-solving skills and the ability to use initiative.

· Excellent IT skills.

· Experience working with others, both face-to-face and via telephone.
