We have an exciting opportunity for a Care Support Worker to join our fast-paced Reablement Team. Our innovative service is about enabling clients to meet goals/outcomes that they identify with a therapist to increase their independence.

The post holder will complete weekly reviews of reablement clients, gathering information for assessment and supporting the service user to achieve their reablement goals. When clients have reached the end of their reablement period/assessment you will be required to assess any ongoing support needs meet eligibility criteria and complete a full assessment & care plan, ensuring a seamless transition for the client over to any long term support.

Alongside Reablement Goals Plans, clients are assessed for small aids and technology aids to support them achieving independence. You will closely with Reablement Assessor, and therapy staff to gain knowledge and skills on these aids and adaptations to ensure a full holistic approach to a client’s care and support needs have been met. Knowledge of low level occupational therapy aids and technology enabled care would be of advantage for this role, although full training will be provided.

As part of a multidisciplinary team including, Occupational Therapists & Assistants, Reablement Assessors and front line Reablement Support Workers to deliver a reablement programme of support to our clients that is person centred, caring and focused on best outcomes for clients.

The role demands excellent interpersonal and organisational skills, a problem solving and positive attitude to ensure clients reach their goals. Previous experience of care assessment and knowledge of the Care Act 2014 would be advantageous although full training would be given.
