This is an innovative service enabling clients to meet goals/outcomes that they identify with a therapist to increase their independence.
The post holder will complete weekly reviews of reablement clients, gathering information for assessment. The post holder will ensure that reablement goals are being worked towards and that levels of support/care are appropriate and tailored to meet client’s needs to ensure they reach their maximum independence. You will be required to assess any clients ongoing support needs meet eligibility criteria and complete a full assessment & care plan, ensuring a seamless transition for the client over to any long term support.
You will also be required to gain knowledge and skills on aids and adaptations that may support a client’s reablement period, ensuring a full holistic assessment for the client’s needs.
You will be supported by a multidisciplinary team including, Occupational Therapists, Physiotherapists, Reablement Assistant Manager’s, and Reablement Support Workers.
The role demands excellent interpersonal and organisational skills, a problem solving and positive attitude to ensure clients reach their goals. Previous experience of care assessment and knowledge of the Care Act 2014 would be advantageous although full training would be given.
This role is currently a Monday-Friday working core hours of 9:00-17:00.
