This is an innovative service enabling client’s to meet agreed goals/outcomes to increase their independence. You will work alongside other Assistant manager’s in the daily running of the service, providing organisation of daily tasks to ensure the service is running effectively and efficiently. You will be required to ensure that the service is meeting the necessary outcomes in line with CQC guidance for providers of ‘Reablement’ services in the community.

The post holder will manage a team of Reablement Support Workers completing supervision sessions, on-patch monitoring and yearly PDR’s. You will be responsible for the management of support staff including organisation of relief cover where appropriate, to meet the needs of clients, in particular by the allocation of tasks, prioritising of workload and re-allocation to assure availability of service capacity. You will be required to assess referrals of new clients into the service, ensuring availability & skill sets to provide the required support to clients as necessary.

You will be required to give advice, and review, the delivery of services to clients in their own homes, ensuring that reablement goals are being worked towards and that levels of support/care are appropriate and tailored to meet client’s needs to ensure they reach their maximum independence.

You will assist the Team Manager by establishing, maintaining and utilising information systems that will inform of performance management of the Reablement service.

The role demands excellent interpersonal and organisational skills.

This job involves working 1 in 4 weekends, as well as being part of an on-call rota providing an out of hour’s service.
