This is an exciting part-time opportunity to contract manage and have commissioning oversight of the delivery of unique Bespoke Residential Care Provision within a specialist provision.

The role will be to independently manage and undertake all tasks and processes relating to resettlement and transition from a Tier 4 hospital settings to bespoke Ofsted registered accommodation and care provision. This will include contract management of a care provider contract and other associated service contracts; as well as working with a range of stakeholders to ensure a successful discharge and transition within project plan timelines.

The successful candidate will work with both the Integrated Care Board and the Local Authority to ensure the terms of the arrangement are fully complied with and escalating issues to the relevant authority where appropriate.

You will be expected to produce and present a range of reports relating to contract performance, advising and recommending developments and improvements to enhance the outcomes of the service and relevant programme; and report to appropriate governance boards

Most importantly, you will ensure that the programme is delivered within agreed cost and timescales and outcomes are achieved.

This is a part-time role over 18.5 hours per week.
