Further Information
We are looking for a highly motivated individual with an interest in helping the most vulnerable excluded members of the community to access advice and support services in order to resolve their longer term housing issues and ensure that they are receiving the support and treatment available to make enable them to make positive life changes.
You will be responsible for:
· Managing referrals in to the project, carrying out assessments to determine clients need.
· Providing support to service users on a one to one basis as required, assisting individuals to resolve problems, agree goals and objectives with timeframes, and reviewing progress with partners.
Ideally you will have knowledge of housing and a proven track history of supporting clients with multiple needs e.g drug misuse, alcohol misuse, physical disability & mental illness. You must have excellent customer service skills and be empathetic. Experience of working or knowledge of social care processes would be advantageous.
