We are looking for an enthusiastic and self-motivated individual to work in Therapy Services within the Home Service Delivery Team.

You will be working with vulnerable adults with disabilities and mobility problems in an Adult Social Care setting. You will be responsible for gathering information and carrying out assessments and meeting client’s needs by providing specialist equipment, requesting minor adaptations to the home, assessing for assistive technology and assisting client to access funding. You will need to be flexible, adaptable and able to work under pressure in this busy team.

You will be responsible for:

. Organising your own caseload.

. Visiting clients who have a wide range of health, social and housing issues in their homes to undertake assessments.

. Establishing how client’s needs can be met by working with the client to achieve their goals and for them to remain as independent as possible in their own home (Clinical supervision will be provided by an Occupational Therapist)

. Making applications for Local Authority grants and alternative sources of funding

. Welfare Benefits checks e.g. identifying shortfalls
. Supporting clients from initial enquiries to completion of work and beyond.

. Dealing with clients, their families, carers and representatives, suppliers, builders and fellow health and social care professionals to deliver services and improves client’s quality of life.
