We are looking for a self-motivated and confident individual to work for Care & Repair, in the Home Service Delivery Team. This job is a combination of the Agency Caseworker and Therapy Support worker roles.
You will need experience helping vulnerable people, collecting and assessing personal information and the ability to respond to the needs of vulnerable clients. You will have a strong commitment to customer care and the ability to work in a team setting. The successful applicant will be need to be flexible.
You will be responsible for:
? Organising your own caseload.
? Visiting clients who have a wide range of health, social and housing issues in their homes to undertake assessments.
? Establishing how client’s needs can be met by working with the client to achieve their goals and for them to remain as independent as possible in their own home (Clinical supervision will be provided by an Occupational Therapist)
? Making applications for Local Authority grants and alternative sources of funding
? Welfare Benefits checks e.g. identifying shortfalls
? Supporting clients from initial enquiries to completion of work and beyond.
? Dealing with clients, their families, carers and representatives, suppliers, builders and fellow health and social care professionals to deliver services and improves client’s quality of life.
