All about the role


We are looking for a new Reablement Practitioner to join our team to support our Reablement support workers working with individuals within our local community.

You will work with our clients through their journey with our service. Starting with their initial assessment within their home, assessing how we can best support them. You will look at things such as what equipment and occupational therapy aids could best enable them to achieve their independence.

Throughout our clients time with our service you will visit them regularly to assess their progress and when their journey with us nears the end you will use your knowledge and expertise to assess if they need signposting to any other services for further support.

As a Reablement Practitioner you will also work closely with our reablement support workers, supporting them and completing supervisions.

As a Reablement Practitioner you will work a 9 week shift pattern, covering the hours of 06:30-22:30, working 3 out of 9 weekends.

This role is fast paced and gives the opportunity to truly make a difference to the lives of many in our local community.


All about you
  •  Experience of conducting assessments including risk assessment
  •  NVQ III or equivalent qualification in health and social care.
  •  Experience in health and social care assessments, enabling clients to achieve/maintain independence, and domiciliary care supervision/management.
  •  Knowledge of care practices, triage skills, risk assessment, CQC standards, and the Care Act.
  • Ability to work flexible hours on a rota system, including early mornings, late evenings, and weekends
