Peterborough City Council operates a reablement service dedicated to helping individuals regain independence in their daily activities. Our goal-based assessments are designed to support clients in achieving their independence, utilizing technology-enabled care devices and occupational therapy aids. We prioritise client interaction while also providing supervision and mentoring to our dedicated staff.

Responsibilities:
• Conduct goal-based assessments for new clients entering our Reablement Service.
• Work closely with health and social care colleagues to support client discharges from acute hospitals and rehabilitation for those living in the community.
• Provide supervision and mentoring to Reablement support workers delivering care and support to vulnerable clients in their own homes.
• Support clients in utilizing technology-enabled care devices and occupational therapy aids to enhance independence.
• Maintain knowledge of care practices, triage skills, risk assessment, CQC standards, and the Care Act.

Requirements:
• NVQ III or equivalent qualification in health and social care.
• Experience in health and social care assessments, enabling clients to achieve/maintain independence, and domiciliary care supervision/management.
• Knowledge of care practices, triage skills, risk assessment, CQC standards, and the Care Act.
• Ability to work flexible hours on a rota system, including early mornings, late evenings, and weekends.
