The Peterborough Older Peoples Mental Health Team is based at City Care Centre, Thorpe Road Peterborough and is part of the multi-disciplinary team of mental health workers supporting older adults who are experiencing severe and enduring mental health difficulties. The mental health Social Workers based within the service are a highly valued group and provide legal guidance, support, and a social perspective to the team, ensuring that service users receive a recovery focused and integrated service. Social Work input is aimed at recognising people’s strengths, enabling people to exert choice and to live fulfilled lives, whilst supporting them to minimise risks to their well-being. There is a strong culture of support within the team and colleagues are willing to share knowledge, support, and experience.

The purpose and role within the team will be varied enabling the person to get a full range of experience across Mental Health Services. The successful candidate will be taking on the lead role in supporting Discharge Planning of our older adults from Inpatient Psychiatric Hospitals in Peterborough and Cambridge, completing initial reviews within the community settings and also supporting and developing a triage and duty function within the service. This is a fixed term role for 12 months.

The essential criteria for the post will require that you have a recognised social work qualification, have a current Social Work England registration and a proven track record that demonstrates your knowledge, skills and experience of social work in the field. The successful candidate must be willing to travel independently.
