The reablement service provides a goal-based assessment to support individuals to regain their independence with their care and support daily activities. We work in client's homes alongside them to support their achievement towards their goals, support the use of Technology enabled care devices and provide occupational therapy aids to enable them to achieve independence.

As well as providing support to staff in a supervisory way, Reablement Practitioner’s offer a great balance between client interaction with reablement assessment, alongside staff supervision and mentoring.

Reablement Practitioner’s will be responsible for the assessment of new clients into our Reablement Service, working from various locations including the client’s home, council offices and your own home!

You will work alongside both Health & social care colleagues to support client discharges from acute hospitals and clients living in the community requiring rehabilitation to regain their independence.

You will need to hold an NVQ III or equivalent qualification in health and social care. You will need to have knowledge of care practices, triage skills including risk assessment, knowledge of CQC standards alongside knowledge of the Care Act. We will also provide you with training to enable you to fulfil this role.

You will be required to work a rota system ensuring that our staff are supported through the working hours of 6:30am through to 10:30pm. There is a mixture of early, late and flexi shifts throughout the rota, giving balance of continuity of staffing levels throughout the week and working day.
